Terms of Service

Fulcrum Massage Cancellation Policy:

I (& my Guardian if I am a minor) understand and agree that a 50% fee applies for any missed appointments or cancellations made within 24 hours.

  1. I understand the massage/craniosacral therapy/reflexology, I receive is for the purpose of stress reduction and relief of muscular tension.

  2. If I experience any pain or discomfort, I will immediately inform the Service Provider so that the pressure or methods can be adjusted to my comfort level.

  3. The Service Provider neither diagnoses illness, disease, or any other medical, physical, or mental disorder.

  4. The Service Provider does not perform spinal manipulations.

  5. Massage/reflexology/craniosacral therapy is not a substitute for medical treatment or diagnosis, and I should see a healthcare provider for those services.

  6. Because massage should not be performed under some circumstances, I agree to keep the service provider updated as to any changes in my health and release the Service Provider from any liability if I fail to do so.

Definitions:

‘I’ and ‘my’ means the client.

‘Minor’ means a client under 18 years of age.

'Guardian' means legal parent, guardian or caregiver.
